This page is broken down into four different sections: Required Forms, Optional Forms, Informational Items and 8th grade Sports.  In each section you will find the forms that need to be printed and filled out in order to enroll in Blue Valley.  The forms are located below in the attachment section. The Registration form can be paid online but will still need to be printed and attached with all of the other paperwork. To view the supply lists for each grade level, go to the informational section.

To actually pay fees, you will need to go to the bluevalleyk12.org website.  Click on the Parents/ Students.  Go to BV Parent online.  There is a direct link below the logo to pay school fees.  You may do so with your child's student ID number or your BV Parent Online password.

6. Activity Fee Explanation and Activity Fee Adjustment Form: Blue Valley has approved a $250 per household cap for the Activity Programming Fee, applied in middle and high school.  If you have paid more than the $250, you may apply for a refund on the over payment.  Please download the form, complete and mail to the address on the bottom of the form.  Do NOT return this form to the school.

If you are interested in becoming involved at OTMS. Please take a look at the PTO volunteer form.  If there is anything you are interested in doing, please complete and return with the above paperwork. 

All order forms (school supplies, calculator and PE clothes) need to be printed off and returned with registration information to the OTMS office.  The transportation form must be completed and returned by all students

1. Bus Rider Information  All bus riders need to complete the enrollment form and return it to the bus company. There is a substantial cost discount for enrolling before July 1st .  If you live under 2.5 miles, there is a fee for the bus.  If you are over 2.5 miles, there is no fee.  For questions regarding your distance, contact the bus company.
