Please complete the three page Student Information Sheet, the two page Student Services Form, the FAN Night Registration Form, a Pre- Enrollment Form and a Math Option Form for each student at the appropriate grade level. Please include a Registration Fee Payment Form with the appropriate payments. If complete fee payment cannot be made at the time of enrollment, a payment plan may be set up with our school bookkeeper.
All order forms (school supplies, calculator and PE clothes) need to be printed off and returned with registration information to the OTMS office. The Transportation form must be completed and returned by all students.

10. Activity Fee Explanation and Activity Fee Adjustment Form: Blue Valley has approved a $250 per household cap for the Activity Programming Fee, applied in middle and high school. If you have paid more than the $250, you may apply for a refund on the over payment. Please download the form, complete and mail to the address on the bottom of the form. Do NOT return this form to the school.
** 6th Grade has no Explo fees.
** 7th and 8th Grade Explo classes: Fees to be paid on August 6 or August 11 when you receive your schedule. Only the Explo Classes listed below have fees.
